Comedians can get away with saying just about anything depending on context and how things are phrased. The reason it works in Always Sunny is because they're not making jokes at the groups they're insulting, they're making fun of themselves of their characters ignorance and prejudices. This conveys a much different message than some conservative comedian who just insults trans people for 20 minutes. Then for me it kinda drops but I still love it. I do get what you're saying though, it's a little surprising and cringey to hear those slurs being used even with the caveat that we know these are horrible people. Like it's still funny and I still like it but it feels more like someone doing a really good job at trying to be sunny rather than actually being sunny like the other seasons.
