Some questions, in no particular order:
  1. Harkonnen seems like a macho/strong society. So why would they drug the Atreides prisoners before they have them fight the skinnier nephew?
  2. Whats up with the cartoonish evilness of all three Harkonnens, where in almost every scene, they randomly kill their own people? How can that possibly be an effective society? Once or twice out of extreme emotions and to show how evil they are via a common trope, sure; but like every single scene?
  3. Why would the emperor come to Dune instead of sending his army and his delegates?
  4. Why is the emperors army just standing in formation waiting around? They had no reason to do this- they werent expecting an attack, they JUST arrived, and there is a storm coming (they should have rather been focusing on creating temporary shelters).
  5. Why did Paul fire nuclear missiles behind the army, instead directly at them? These bombs dont seem to be world-ending powerful, so strategic missiles against the well grouped army seems to have been a better option.
  6. Why would the emperors army decide to go for a charge in an open field with sand everywhere, instead of falling back and creating a solid line of defense?
  7. When the line was lost and Paul and his folks got into the emperors chambers, why would the emperors personal guards of like 10 or so decide to walk straight into the door that they cannot even see (because of lots of sand) instead of falling back and bolstering the final defense soldiers?
  8. How come majority of these soldiers seem to not have any of those shields up, and the fremen are tearing through them so easily? The first movie made it very clear you have to attack in a specific fast-but-slow method to pierce those shields; its very clear the fremen are not fighting like this, nor were there any scenes that showed them being trained to fight like this.
  9. Why would the other great houses not agree to the ascendency of Paul, when hes marrying the emperors daughter, and have spice production (a critical component of the entire galaxy) literally under his control?
  10. Paul releases the fremen to go attack the other houses in the air/space. How do the fremen know how to pilot the emperors spacecrafts?
  11. Why was Zendaya so bitchy about Paul using his messiah-like influence to bring the fremens together to fight the foreigners? It should be one of us that leads arc makes no sense since clearly they cant do that.
  12. I didnt understand the significance of walking through the desert in that dancing way. Why walk like that? If its to avoid the worms, how come they dont always walk like that?
  13. If the worms can be summoned so easily, be driven around like trains and devour everything in their path, are bulletproof, and arent harmed by missiles, why havent the fremens used it as a weapon?
  14. If the emperor can very easily tell the south has human activity, how did the harkonens miss that?
  15. Why was the emperor so quick to get rid of the harkonens (destroying Barons life support and his spine thingy)?

This is what I wanted to know. I actually asked each OBGYN if I can be in the surgery room should we opt for the circumcision, and the answer was no. From my online research, there are three main methods for pain- ones do nothing, another is an injection, and the last is a cream; the hospital were choosing to deliver at is the cream apparently.

That's not the case for our hospital network. We're told we have to sign documents and determine if we want to or not before the baby is born. We apparently can't change our minds after the baby is born (but can change our minds during labor); if we signed that we're not doing it, but changed our mind after the baby is born, we have to do it out-patient and not part of the OBGYN (but not vice versa; i.e., if we signed we're going to do it, but changed our mind after the baby is born, they'll have us sign to override our previous decision). Not sure why they do it this way...

And to answer this question logically, I'd have to compare two sets of data- probability of complications due to circumcision at birth vs. probability of complications of having a foreskin. I have no sense of these likelihoods... as I mentioned, the older OBGYNs seem to believe circumcision is better, with the younger ones seeming to believe it's social preference.

So here's my thoughts:
1) Assume the fact that getting circumcision at day 2 of life = no pain is true (per OBGYN's statements that babies don't react).
2) If you need to get a circumcision later in life, the surgery is significantly more involved, and will have higher chances of complications.
3) If you don't have a foreskin, complications with foreskin issues go away, and thus you would never have to deal with Item #2 above.

So the question for me is- is it worth getting circumcision when it's easy to do to avoid complications with foreskins later in life?

My firstborn was a girl and she had severe tongue tie. On day 3 of her life, we went to an oral surgeon to get it corrected. Once we got into the room, which looked like any pediatrician room, the oral surgeon came in and asked for final approval. Once we approved, he took her, turned 180, brought her over to an examination table, opened a brand new knife out, opened her mouth, and just cut the tie! No creams, no shots, nothing. I was thinking what the fuck just happened. Our baby didnt even wake up, didnt cry, there was no blood in her mouth, and it was all over.

I later found out for tongue tie, if you do it later than a week or so after the babys life, its a lot more involved. This experience is making me believe the OBGYNs statement that there is no pain for a brand-new newborn.

We're having a boy, with a scheduled delivery date of May. We're starting to be asked by the OBGYNs if we're planning on circumcising or not. We spoke to 4 different OBGYNs and here's the general medical consensus from them:
  • Circumcision will be done by the OBGYN the day after the birth.
  • The operation: Baby will be given a pacifier dipped in sugar water, and a special swaddle will be used that is only for the upper half of the body. The legs will be held down by straps. The penis will be sterilized and then a numbing agent will be applied onto the penis (it's a cream-like substance, and not a shot). After waiting ~5 minutes for the numbing agent to take into effect, a tool will be placed over the penis to guide the cut, cut will occur, then clamped back. No stitching is required. Apparently, the most bleeding you'd expect is also a drop or so; and most babies undergoing circumcision don't even cry or show any discomfort. This seems hard to believe, but all 4 of our OBGYNs said this.
  • Recovery: Gland will be swollen and may start crusting. No baths or showers allowed until fully healed. For the first 5 days, at every diaper change, use warm water and a sponge to lightly dab the penis, and cover with Vaseline to prevent sticking onto diaper. After the first 5 days, and until fully healed, wash at least once a day. Once fully healed (estimated to be between 7 to 14 days), bathing is acceptable.
  • Complications: The surgery itself- extremely rare; if anything happens, follow-up surgery may be required, but not by the OBGYN, but by a urologist. Most likely complication will result in follow-up surgery within the first 5 years due to skin fusing incorrectly to the gland. Recovery- Bacterial infection is extremely unlikely (also hard to believe???).
  • Benefits: The older OBGYNs seem to point out that there are a lot more medical benefits; they point out that having a foreskin could lead to problems with retraction due to improper "un-fusing" of the foreskin with the gland or the foreskin being too tight, infections due to poor hygiene, and higher STD rates. The younger OBGYNs seem to point out that it's largely social and visual preference. Regardless, all OBGYNs stated that if you don't do circumcision within the first few days of being born, the procedure gets significantly more complicated and will require general anaesthetic.

Personally, I thought circumcision would have much higher chance of complications and the baby will be tortured/hate it. But all OBGYNs that we talked to say that most babies literally sleep through the entire operation and the ones that don't are just fussy (and not screaming/crying). Given that, I feel like doing it is a no-brainer, since complications with a foreskin later on in-life is a thing.

The only reason I don't want to do this is because we are able to travel for very cheap, so miles/points aren't really that valuable. For example, we have a yearly-use timeshare property that was gifted to us (where we just pay the annual maintenance fee and no mortgage), which grants us access to the RCI network of cheap resorts (e.g., ~$300/week all over the US for timeshares that are vacant), we get our Hilton hotel stays for extremely cheap under government GSA rates for personal travel, and any flights we just book the cheapest possible flight (vs. being locked into one airline for loyalty).

I have all sorts of non-annual fee cards, like Chase Freedom, Chase Flex, Discover, etc. I used to play the 5% cash back rotating categories game, and then just got tired of keeping track and started using the Double Cash for everything except gas, restaurants, and Costco purchases (these three go to the Costco card).

I'm sort of now thinking of getting back into the 5% rotating category game, or even getting a premium card (like Chase Sapphire Reserve) and locking into Ultimate Rewards program.

I cant think of one retailer that will allow you to return, for a full refund vs exact exchange, a new game / game console. Financing, also, will result in me buying a PS5, which I dont want to just for one game. I searched Grover, and seems like thats not a US thing. I found a company that does something similar, but charges $30/week with a 48-week agreement. Thats INSANE!

I just got to the part where someone dies, and it makes no sense to me.

SPOILERS

How does Chidori and Junpei fall in love so quickly over absolutely nothing? What did Chidori even do to make Junpei love her so much, except look like some weird maid sitting on a bench drawing stuff? I get why Chidori liked Junpei, because Junpei kept her company when she was in the hospital for months, but there was no reason for Junpei to do that in the first place, and didnt make much sense for Chidori to fall in love with him so quickly either.

Temperature ranging from freakin' 30F lows to 64F highs... need to use furnace overnight, and then when the sun comes up, the house gets to be like upper 70s because of the latent heat and added heat from outdoors, necessitating either opening all the windows or turning on the AC. Feels like such a waste of energy.

1. It's not about "patient turnover", it is about patient hand off. We have the responsibility to signout or "give handout" of the patients we take care of (like giving the summary of the patient, what needs to be done tonight, etc). Often times during this process, some things get lost in communication which can lead to delay in care or a bad outcome. If you shorten the number of hours per shift for each resident, then there will be more sign outs, and more information gets lost in the process.
2. The culture nowadays is shifting to less 24 hour calls. But it really does decrease the amount of learning and clinical training if they don't do 24 hour call. You really do in fact learn more and become more independent and build confidence in your skills if you work at night time because of decreased support staff. You will still have one supervising attending watching you, but you are limited on your resources to call, so you triage and figure out how to prioritize. Residents learn on their feet when they do this and it is what makes them competent physicians. Nowadays these residents don't even know how to order the proper dose of antibiotics and they waste my time and call me asking what dose to give, instead of spending time to look it up.
3. Getting rest or sleep really depends honestly. If you have a quiet night with nobody calling you, then good. If you have to see and deal with a sick patient, then you work it through.
4. Frequent hand offs lead to poor continuity of care for residents and overall worse training.

Just a reminder that 24 hour shift doesnt necessarily mean no sleep or 24 hours straight of work though you can have days that are very busy. When I was in the military I regularly had 24 hour duty days. Similar situation.
Military is just "hurry up and wait" tho.

Well actually, one justification I read is that it reduces mistakes due to reduced patient turnover; in other words, info missed during increased number of patient turnover may harm more than a doctor being tired. No idea how true this is though.

Was at an OBGYN visit with my wife, and the OBGYN casually mentioned that she delivered 5 babies her last shift. I asked how long her shift was, and she said it's 24 hours long. I asked how often is her 24-hour shift, and she said 4 times a month; could be once a week or more than once a week, it's really up to how scheduling works. Even so, she has to work three 10 hour shifts per week outside of that for patient care. She then said she's been doing this for 26 years, so she probably delivered over 2,000 babies and laughed.

That's fucking NUTS! I Google'd it, and saw this recent Reddit post that confirms such crazy shifts, all over the world (there are comments that say that in Europe, Germany, etc. they also do this):
