Granted, but he came very, very close to succeeding in his goals at times. The only things that stopped him were his utter fear of Luke and the fact that even as a Sith he had trouble fighting his sister, and even stopped fighting her to protect his daugher.
Death is nothing to the Sith. It's expected, especially with the Rule of Two. The way they look at it is if someone kills you then clearly you weren't strong enough.
