Sometimes when they go to run your insurance for medications and stuff, whoever took it down didn't write it correctly and it makes it pop up as you not being an active member of said insurance.

Other times it's unclear what forms they should be using to get your medications okay'd. Your card will say a bunch of stuff, but it'll just go through Caremark/Escripts/Medicaid. Its not super cut and dry sometimes.

Generally, if it's there's issues theyd just ask to verify the info on your forms though. More often than not it's something dumb like an incorrect zip code or something. There are definitely folks that let their insurance lapse and still try to use it though.
