Whoa I never realized you got new enemies with high insight. I generally kept it high fortunately. But uhhh once you get to a certain area I highly recommend nuking it because frenzy is a bitch there. You'll know when you get there.

Anyway, I just recently played this for the first time too! Think everyone has covered the points I would. I would just add that the missable areas are so very easily missable, but none are permanently missable. So look up a guide before new game plus if you don't find them. They're pretty cool.

Also also play the DLC because it's amazing and upgrade your whirlygig saw (pizza cutter!) asap when you find it in the DLC. Best weapon for the insanely hard final boss with its OP stagger. And it's just ridiculously fun

Also also also some of the highest level Caryll Runes can only be found in the highest difficulty chalice dungeons. But you can also check the Bloodborne wiki for passcodes to enter for those chalice dungeons so you don't have to stumble upon them randomly. But you do have to have a root chalice for that type of dungeon to be able to search its glyph code

Okay one last thing. If you ever want to farm, Lecture Building 1st floor is the perfect place to do so when you get there. Can get a ridiculous amount of Blood Echoes real fast. Even if you don't like farming, still a useful tip if you want to just farm to buy blood vials. You will probably have to buy some for some of the bosses.

E: Also high insight does increase one thing: beasthood resistance
