Unless I'm grocery shopping (where there can be variance due to the weight of items) I will typically know what I expect my total (after tax) to be within a few cents and if my total is ever different from that expectation I'll say something. With groceries I have a ballpark and I'll speak up if I end up an unusual amount over. This might happen if the cashier punches in the wrong item code on a bulk item that is being weighed such as ringing up 5 lbs of peanuts as 5 lbs of cashews which would cost over 3x as much. If my total is a bit low I'll chalk it up to good fortune and say nothing. The very last reason I would ever want to hold up a checkout line at a grocery store would be to ask them to charge me more money.
