I'll usually point it out unless the cashier is being stupid.

I used to go to a supermarket that had a water bottle filling station and I'd fill 9-12 three-gallon bottles at a time. They always charged by the gallon, not by the bottle. When I'd get to the register I'd say something like "I have 9 three-gallon refills, that's 27 gallons." Roughly half the cashiers would reach for a calculator even though I just told them the answer, and one fourth would either take my word for it or do the math in their head. But the remaining quarter would ring up 9 gallons even though I just told them 27. And when they did that I'd just shake my head and not say a word. If the store is going to hire a cashier who is too dumb to listen or do math, then they deserve to lose money.

It's not like the store was paying much for the water, it was literally tap water run through an RO filter so it couldn't possibly cost them more than a couple cents per gallon. Even if I was paying only 1/3 of their actual retail price they still made an excellent profit.
