OK, but hospitals have an obligation to save EVERY life that they can.

And the hospital still can. True that doctors are supposed to adhere to the Hippocratic Oath, but that doesn't mean they're required to risk wasting a healthy heart on a patient who can't make the effort to ensure they're the best candidate for that heart. They didn't deny the patient wholesale. They only moved him down the list.

When someone seeks a gastric bypass, which in some cases is a surgical procedure crucial to improving one's life expectancy, they are often tasked with making changes to their diet and meeting weight loss milestones before they are allowed to get the surgery, in order to improve the odds that the patient will thrive and succeed and continue making better health choices post-op. This heart transplant is no different. They want the dude to live a long life with the heart he's being given. The least he could do is improve the chances he won't drop dead of covid a year after receiving the heart.

Except this one decided that this man should die, literally signing his death warrant just to grandstand for a vaccine.

The patient signed his own death warrant through his failure to make himself a prime candidate for the transplant.

It's less "grandstanding for a vaccine" (phenomenally dumb take btw) and more standing up for making certain the organ goes to the best possible candidate who will appreciate the opportunity and make the effort to improve their own life to make full use of the heart, hopefully living out a long, full, and content life.
