He doesn't understand what the 4th wall is then. Cause having someone from behind the scenes as an extra isn't breaking it. It's a subtle nod towards it but its not like he is in the movie as Stan Lee, he is just some random individual in the Marvel world. Heck, in Iron Man he was Hugh Hefner!
The costume comments are hilarious because they reference the source material and how it relates to the world of the marvel movies.
