I tried a bunch of different set-ups and she always died in the Extreme boss' first UB, but the timing didn't change much so all of them could easily have been during a debuff. All I remember for sure is that she was always the only one to die during that attack and borrowed Mokotos would survive, just like mine did before ascension. Often ranking them up or giving them too much gear impacts all that as well.
