In 20 years you'll swap ages with a random person who's the age you were when you accepted the offer. So if you're 38 now, at 58 you'll age swap with a 38 year old. This makes the deal literal hell for anyone who is already older. I wake up with aches and pains as is. Being trapped in an eternal loop from 47 to 67 feels like it would be a fate worse than death. Even before we start factoring in stuff like watching all your loved ones die or the government hunting you down to dissect you.
