When I was in 7th grade at my karate school, one of the kids brought a 3DO and we all took turns playing Gex. In one of the first levels, there's a spring and a wall and everyone passed the controller around trying to get through it. They all jumped into it even when I told them that's not gonna help.
It gets passed to me, I walk past the spring, collect the item, go back around, and do it again before handing it back.
Caught all the non-event Pokemon in SS (including Mew from Ranch and Manaphy from Ranger). Got all the Unova pokemon too, but I'm waiting for 3rd version to transfer them all to a Gen 5 game. I also got 100% Support Convos in FE7. Now I feel like a huge hypocrite when I say I don't like grinding <_<
Beating the optional Vanitas battle without using cheap tactics like hiding behind the rock at lower than level 50.( As Ventus) That's the most recent and first to come to mind.

100 Super Jumps in SMRPG? Beating the 8x Pringer X battle in Disgaea 2: DHD? (Though this one is really more a matter of having the will to build a unit/team to handle it)

1% run in Metroid Fusion All Pokemon in Red, Crystal, and Pearl I guess beating FE7 in 178 turns with a drafted team, but it was HNM so I feel like that turn count's probably pretty bad overall

100% and all Platinum medals in Time Splitters 2 100% and all Platinum medals in Time Splitters Future Perfect 100% F-Zero GX No death Contra run. (although I do this pretty regularly, I always see people complaining about the difficulty, so I just mark it as an accomplishment) No death Life Force run. (same as above) Only 36 deaths in Jumper (this game: http://mattmakesgames.com/games/Jumper.zip) Completing a lot of the really hard songs in DDR and ITG.

In 3rd grade, at my before school program, we had an old Apple II e with Winter Games. One day there were five of us playing it. A 5th grader named Matt knew that I needed to beat him in the final event, Bobsled, in order to win the Olympics. Bobsled is a simple matter of pressing left and right at the correct times to navigate the turns without flipping, but timing is critical.
Matt finished his third and final attempt. It was better than my previous mark. With my last chance to beat him, I started down the course and...he came up behind me and put his hands over my eyes. I didn't have time to shake him off me, so I ran the course's timing completely from memory, and got the best time out of all of us.
100%+platinum in Atelier Rorona: The Alchemist of Arland Beating Lucifer in Shin Megami Tensei: Devil Survivor (plus all the endings) Caught all 493 (at the time) Pokemon in Pokemon Pearl

Oh and a few other proud accomplishments, if you want some more concrete stuff...
I won a gave of Civilization 1 on Emperor before 3000 BC. On another game, I covered the map with cities so thoroughly the game's memory glitched, but before it did I had reached well past Future Tech 1000, and had at least 100 turns left to play.
During BC era WoW, I solo leveled a character to 70 in 108 hrs. Allegedly the record for doing so was closer to 120. (Not that we'll ever know, because there were players using outside assistance to level much faster.)
I completely cleared the enemies in the second level of X-wing, where tons and tons of enemies warp in and you're just supposed to ID them in an A-wing. I had to solo kill a star destroyer and a frigate, plus all their fighters, as well as a small fleet of corvettes and other light capital ships...in an A-wing. I came away from that mission with something obscene like 300,000 points. The highest piece of the Kalidor Crescent only requires 20,000.
My absolute proudest moment ever in gaming was a time when, leveling my second character in EverQuest, I got a team of newbies through an absolutely impossible set of circumstances as a cleric that served not just as party healer, but crowd controller (because I was the only caster) and battlefield strategist. While staring at a spellbook to regain mana. And telling people what to do in a text-only medium where the game also uses the keyboard. In a situation that is even more ridiculous if I would tell you the whole story, but unfortunately too much backstory of EQ mechanics would be needed to explain fully. Let's just say it involves fighting off continuous waves of backspawn for about 40 minutes, and juggling party health versus a mana bar that was depleted 15% of the way into the fight, and getting out without any casualties.
I have to admit, it's almost embarrassing to say that my finest hour was while playing EverQuest, of all things. However, the degree of awesomeness I put on display that day has never been equaled. At least, not by me.

When I was like 13, I asked my grandmother to surprise me for Christmas. One of the gifts I got was this neon dolphin light that was actually pretty cool looking. Just a year or two later, my cat knocked it over and shattered it.
