Hit it with Saboteur effects and then Stagger it as fast as you can.

On form 3 you have to stagger him before you can do any damage. My advice is to have a RAV/SYN/SAB set up for a few turns and quickly buff yourself/debuff Orphan, then after that switch to COM/RAV/RAV (or RAV/RAV/RAV if you have it) and hit him until he staggers. Then either stay on COM/RAV/RAV or COM/COM/RAV (or COM/COM/COM if you have it) until you kill him.

You shouldn't really need to bother healing yourself on form three. Just worry about buffing/debuffing/staggering.

Grats on the final boss. I had to play the second stage about 10 times before I finally beat that part. Every time I limped along to the point I thought I maybe had it, he would insta-kill my lead character.
