Not sure how it works in other states but in mine all you have to do is go up to the pharmacy and ask for it. They write down your ID and thats it.

It's behind-the-counter at the pharmacy. You have to present your ID to obtain any product containing PSE as per the Combat Methamphetamine Act of 2005.
