Why do people do this?
I understand and accept that tipping is a thing in the North America, and it's not gonna go away. A lot of people make sure the tip reflects how satisfied they are.
For example: OK quality = 10%ish, Excellent quality = 20%ish, Shit quality = 5%ish

Why do people tip at all when the overall experience is terrible? I'm talking about when the restaurant in question put zero effort in making sure you have a good meal, like the food tastes awful and the staff is rude. There are plenty of places like that, yet people still tip at them.
Yes, a lot of waiting staff is largely dependent on tips for their income, but that doesn't mean that the effort they put into it doesn't matter.
Wouldn't tipping without the restaurant having earned it kind of defeat the point of tipping being an incentive for good effort on the restaurant's part?

I'll tip if the server at least appeared to be trying. Sometimes shit happens. But if it's absolutely awful I dont mind skimping.

My metric is how many times was my drink refilled, since I can go through at least three glasses of water per meal. As long as they can do that, I'm pretty happy.
