My answer was always something along the lines of:

"I'm looking for opportunities to grow. My current employer has provided me with a great deal of growth and knowledge within my career, however there are very few opportunities to grow within the company. At this point, I feel that I have outrgrown the position and am looking for a new challenge."

Pay is always negotiable and if it isn't why even have an interview?

In the past when head hunters call me that's the first question I ask them "what is the pay range?" Because there is no sense wasting peoples time if the pay isn't there.

And I never give out my current wage. Why on gods green earth would I tell them how much I'm making?

What they want to hear is that you're going to be a good fit for the team\culture and are a good investment for them. No one wants to employee someone and train them, and waste time and money only for them to be a job jumper.
