Either, or. Really depends on what time period is being referenced, later in life both could be equally acceptable. He was adopted by Worf's human parents for a couple reasons, likely most importantly regarding the name change was Worf wanted to distance his son from the dishonor of his discommendation. It wasn't until some time later, when Worf helped ensure Gowron's status, that their family's honor was restored and Worf's brother given a seat on the High Council.

Wow all of that is wrong so I deleted it. I feel ashamed.

Technically, Worf was adopted by the Rozhenkos and technically should be Worf Rozhenko as well (earth customs to take the adoptive family's last name), but instead prefers to identify with his Klingon heritage.

Alexander (as a child who lived on earth) would have probably used his Rozhenkos name as well. After he joined The house of Martok? There's really no reason why he wouldn't be called Alexander, son of Worf.

Also by this point, Worf has restored his family name (before the house being completely dissolved again) so there's no point hiding the fact that you're the son of one of the most well respected warriors in the empire after both of them joined The House of Martok.
