I'm level 77, and it's been almost a year since I've played it.

Am I better off starting a new game or what?

There's never a good reason to start a new game in that game.

You'll be fine. They're independent stories, though the Far Harbor DLC will make reference of decisions you made in the primary story.

Do yourself a favor (if you care about getting all the achievements) and make a separate save file before the point of no return.

You might wanna bump up the difficulty if you haven't, you are extremely overlevelled for the content.

The game makes you really clear when you're at that point.

Eventually you do have to side with a faction, and when you do there's a prompt that says "once you do this you can't work with the other factions any more."
When you get there just say "no" to advancing and make a backup save.

Saves you from having to play through the entire game again just to get the other endings.

Never gave a fuck about achievements/trophies, but

No problemo. I decided to do that since the game felt like a chore by that point. I somehow also managed to stomach the "get 100% happiness" achievement >_>.
