You auto-accelerate and basically can't go off course and it just shoves you back on track. Items are used automatically upon grabbing another item box if you haven't used them yet. You gotta basically figure out a good build to get a lot of points. How well you do don't really matter as much as getting a lot of fever time and hitting a bunch of cpus with items.
