Are Misako and Kyoko brain-damaged?

Prior to fighting NOIZE, Misako mentions her memory is bad because she gets hit a lot.

The ending of the game indicates that Riki and Kunio are, in fact, NOT dating the main characters. They went on an obsessive tirade because they were horny for boys who aren't into them.

So like are they seriously concussed and mentally fucked up from taking too many punches to the head?

Its a bit of a series in-joke, in the majority of the games Kunio and Riki are romantically linked to Mami and Hasebe, not Misako and Kyoko. (Though usually Hasebe is just pining after Kunio rather than actually dating him, or sometimes shes with another guy entirely like in River City Ransom) Theyre just pathetic weirdos who got so obsessed with the boys that they forgot they arent actually dating them. The bonus ending if you destroy all the statues helps a bit to explain all that.

Huh, apparently part of a patch earlier this year slightly altered the ending so they seem less pathetic. That was a part of the game's humor I really appreciated! A lot of players got mad at it all, though.
