They decided to turn him into Jaws and make him show up every 3 minutes because they felt it would make him appear more sinister and evil. Midgar is 10% of the original game roughly. You dont even see what he looks like iirc until a long, playable flashback a little later. Yes, you leave Midgar. The city is only about 4-5 hours long in the original.
