We definitely do meat with our spaghetti. At least my family always has cooked up some hamburger meat before throwing that and maybe some mushrooms into the sauce. My wife is a fan of premade meatballs so we go with those now.

We generally have a meat sauce in this family. Usually some hamburger but I've done shredded chicken before (though chicken not very often).
I also made hand made meat balls twice this week. I made like 100 frickin meat balls this week.

I think it's just Google images food being disgusting looking in general.
My favorite pasta dish is a baked ziti/rotinii with sauce, cheese and meat (usually ground sausage and/or ground beef), but it is usually just regular pasta sauce, and the meat is added separately, but the meat is not in the quantities that I find when looking up "spag bol" online.

I also like a regular baked pasta. Take some cooked ziti,layer it with a mixture of pasta sauce and sliced sweet Italian sausage and top it with some cheese, and bake it for a few minutes.

I think that Spaghetti in general is a poor man's pasta, and generally don't like it.

Keep in mind, that I don't like a lot of foods, so I'm not really a good representative of a "regular" American. I don't think I've ever just eaten ground beef before, it is usually mixed into something (like pasta), or cooked into patties or balls. Don't mIncE mEaT me, I'll start calling Hamburger Patties, .

Even when I'm too cheap to buy meatballs (or too lazy to make them), I'll brown ground beef and toss the chunks in there. Just eating spaghetti with sauce doesn't feel right to me.

That would be why I capitalized meat, so yes.

But generally speaking, American spaghetti doesn't have both. Like, if you have spaghetti and meatballs, you won't also have meat chunks in the bolognese, and vice versa.

Yes we do. It depends on what people like of course, but we do have spaghetti and meatballs and spaghetti and meat sauce. Sometimes with seasoned ground beef, sometimes with ground Italian sausage.

I interchange hamburger beef and ground beef. It's ground beef. I don't literally take premade hamburgers to make a meat sauce.

No one here says spag bol, that is likely the source of your consternation. In America, meat is already understood to be in everything, so it's just known as spaghetti. If you want meatless spaghetti, you'd specify it.
