It is a slang term for a situation that is chaotic or confusing, possibly due to poor or misunderstood instructions.
The term goes back to the early 1900s, and is alleged to have originated when a ship run by British officers and a Chinese crew practised a fire drill for a fire in the engine room. The bucket brigade were to draw water from the starboard side, pass it to the engine room, and pour it onto the 'fire'. To prevent flooding, a separate crew was to ferry the accumulated water from the engine room up to the main deck and heave the water over the port side. The drill had previously gone according to plan until the orders became confused in translation. The bucket brigade began to draw the water from the starboard side, run directly over to the port side and then throw the water overboard, bypassing the engine room completely.

That's where I've heard it before! Never done it myself, but my mom and aunt told me it was a thing the youth groups used to do.

I've heard of it. Many decades ago, like the 1950s or so, teenagers in cars sitting at an intersection used to shout "Chinese fire drill!" and everyone would exit, run around the car, and find a new seat to sit in before the light changed. I don't think kids are that creative anymore.

Up until about the mid 1990s, HS kids and some adults would stop at a red light and everyone would exit the still running car and run counter clockwise to the next door, enter and try to be seated before the light changed. This was also known as a "Chinese fire drill".
