The free upgrade contains 4 Creations: Rare Curios, Saints and Seducers, Survival Mode, and Fishing.

The $20 upgrade to the Special Edition contains all Creation Club content released for Skyrim to date, along with all the unreleased Creations made after the CC went on indefinite hiatus in 2019.

Creations are basically micro-DLCs (or paid mods, if you want to think of it that way) that add either original or throwback content with short quests associated, including weapons, armor, player homes, creatures, ingredients, things of that nature. There's no voice acting in Creations beyond recycled stock lines, so the quests are typically limited to notes / journals for progression. Some Creations are great, some are bad, most are mediocre. The most offensive thing about the CC was the price per Creation (and that's still true if you're buying them individually for whatever reason), but for the amount of content you're getting with full AE upgrade, $20 is pretty reasonable.
