Not necessarily. It could just mean they have other candidates left to interview and don't want to make a final decision until everyone has been evaluated.

They may have other applicants or another stage of interviewing. When I'm looking for someone I go through 2 rounds of interviewing.

No. I start a job on Monday where that happened. Getting hired on the spot after an interview is rare outside of very low paying jobs like fast food or stocking shelves in a grocery store, in my experience.

Nah. As a job interviewer, that's my go to response for everything short of a candidate absolutely hitting it out of the park, which has only happened for a small number of jobs for which I've held interviews.

No. At my current job I got some variation of that after each of my four interviews. Very rarely does any single person have the authority to hire you on the spot. They have to talk with others and discuss logistics for next interviews and what not.

It just means they have other applicants to get through before they make a final decision, it's rare to be hired on the spot (usually means they're desperate so might not be a good sign anyway).

They say that in pretty much every interview, so it doesn't mean that you didn't get the job. An interviewer said that to me once and then called me again a day later to tell me that I got the job

That's standard interview language.

I would suggest to always follow up (unless you already know the answer), do you have an estimated date when I would get a response?

Then after those X days have passed and you havent heard back, follow up via email/call/your recruiter point of contact.
