Well more of a Gotham Game bevause Batman himself is not the main character possibly not even playable or in the game at all.

The premise is the Bat-Family is seemingly(possibly truly) all killed. Gotham is overrun by villians. Fear Gas, Joker Venom are all over, horrible experiments from some of Batmans more (mad)scientifically minded enemies and psychopaths from Arkham run amok in the city.

Scarecrow, Joker, Man-Bat, Clayface, Killer Croc and other monstrous or truly deranged villians take center stage over the more normal(I.E mostly sane merely criminal as opposed to outright insane), sympathetic or misguided ones like Freeze, Ivy, Penguin, Harley etc.

You play as a mostly ordinary citizen trying to survive armed with only your wits, fists and sometimes lite arms, discarded/lost scraps from heroes/villains or melee weapons.

if the city is taken over by villains, how much time do they really need to kill off all the civilians and after that, who would they even terrorize

I mean its not like every civilian instantly fucking dies the instant Batman leaves the scene. This would be very shortly after the aftermath of Batman being gone. Most survival horror games take place in a short time frame.

Also there would be a lot of criminal infighting and many of Batmans insane rogues experiment on people or play with their victims. Some of his Villians like Penguin are more like Mobsters then murdering loons.

And the police exist and have resources. Obviously they cannot keep the city as a whole under control but they can hold certain areas for awhile. Perhaps even cut deals with some of the saner villians like Penguin. Desperate times.
