Without using glitches, playing as the devs intended.

My belief is that you need the hookshot to be granted access to levels 3 and 4, and so on. Unless I can find one of those warp points earlier than I am aware?

Don't think you need the Hookshot for 4, but you might need it for 3 (Definitely need it for 6). Should also put 3 between 4 and 5, and 4 after 3 connecting to 5, since the devs probably intended you to have the Fire Rod (given that they locked the very first door in 5 behind having either it or Bombos, and I'm pretty sure Bombos was not what was intended).

This. When in the Light World, enter the Lost Woods and exit the woods via the south-central entrance. You'll find a Magic Hammer post that can be pounded down to access a dark world portal. The large Titan's Mitt rock fools you into thinking you need the mitt to access that dark world portal, but you don't need it.

In addition, both the Fire Rod from dungeon 3 and Titan's Mitt from dungeon 4 are required to enter dungeon 5.

You can do 2,3, and 4 at the same time.

6 can be done any time after 4 using the boots to bounce over the gap in the first room but isn't exactly dev intended. Not a glitch but not intended either, just an odd bit of physics

5 needs the items from 3 and 4. Intended it needs the item from 2 but this can be skipped with some backtracking inside the dungeon

7 requires the items from 1, 3, 4, and 6 explicitly but not 2 iirc.

Thinking about it, I'm not sure the hookshot is REQUIRED anywhere outside of 2 but I might be forgetting an oddball room somewhere
