There's a part where if you don't click pick up fast enough, you end up kicking the Elden Ring into a volcano and you can't finish the game.

You can miss parts of some quests just by going to places too early, but usually you can still get the reward except in the large jar being stuck in the ground rewards, but they are often nothing you couldnt get elsewhere.

The only other way is to kill npc's that can screw up any quest line most likely.

A lot of the character side quests are fucking horrendously set up. Without a guide, 100% you'll miss a good chunk of their early stuff, and if you ever start their later quest overall timeline wise, you'll be locked out of anything you could do with them prior to that point. There aren't a whole lot of hints as to where you can start some of the quests either

But if you try to do them all, you'll lose interest in the game like I did lul

as soon as you meet an NPC, just save yourself some trouble and bullshit by looking them up on the wiki. it'll show their order of locations or any points of failure to avoid. it's extremely stupid, but the game is too long to replay the entire thing over just to do a few quests you didn't even know you fucked up

While not really a fuck up and more like a "easy to get lost" situation, there's a point where you're told to look for a character named Blaidd and all I can say is that he's not where you're told he is.

One of the merchants is currently dressed up like Santa, as is his mule. He is right there near the starting area. Not a quest but he probably goes back to normal pretty soon
