Spider-Man pulls his punches by a lot. If he really wanted to, Spidey could have easily reduced Otto to an ugly smear on the pavement. And even then, thanks to the aftereffects of the nuclear accident that fused his tentacles to his body, Otto wasn't able to properly heal from the various wounds he incurred in superhero fights (and not just from Spidey, either; Otto once got his ass kicked by the Hulk), and so his body was slowly breaking down. This was why Superior Spider-Man happened, and why Ock is now a clone driven by an AI backup of his mind rather than the original. ... Copied to Clipboard!

My guess as to how this is possible in-universe is that Otto had a hidden Peter-clone with Otto-AI backup ready to go (just in case he died doing the superhero thing) before Mephisto made him forget all about having once been Spider-Man, and that it was activated by Otto failing to reset a dead-man's-switch for it. ...
