That's just a dumb requirement to get a full ending.

Usually it's the other way round

Not true in a lot of cases.

Most games imported from Japan to the US tended to have the difficulty in some form scaled up, and this practice continued for years before being dropped in the 7th console gen.

I can tell you Capcom was especially notorious for this for the longest, as even in earliest Resident Evil games that had difficulty settings, the actual "easy" mode for the US versions is outright removed.
Devil May Cry was especially notorious for it (though at the time of the first three games, no one realized), as the true Easy difficulty was removed and "Normal" became the Easy selection, making the player stuck with actual Hard and Very Hard difficulty modes from the get-go.

Hell, let's go even further back
The Konami X-Men and Simpson arcade beat-em-ups in the Japanese version had a lot of collectables to help players out, and in the case of the X-Men beat-em-up, using the mutant power feature didn't drain both your life and mutant meter, and you could pick up refills for your powers.

We had games that were made harder, but also games that were made easier (remember Final Fantasy 4?).
And then we had games that didnt know if it would be made harder or easier.
Like Alien VS Predator on Snes.
JPN: Walk slow, dash at twice the speed.
Int: Walk is the JPN dashing speed.
JPN: The disc and spear can only be thrown once.
Int: They can be thrown 6 times.
JPN: The cloaking device lasts 44 seconds.
Int: It now last 16 seconds.
JPN: Novice gives you 112 HP, Standard gives you 80 HP.
Int: Novice gives you 128 HP, Standard gives you 96 HP.
JPN: Normal attack power.
Int: Your punches are roughly cut in half. Your shoulder laser is also weaker.
JPN: The special attacks. Using them drains your HP. Weak laser takes 5 HP, Strong laser takes 20 HP, Full laser strike takes 40 HP.
Int: Weak and Strong laser drains no HP. Full laser strike takes 12 HP. Your special attack power bar takes one second less to charge.
So basically you walk faster, deal less damage, got more HP and most of your special attacks doesnt drain your HP.

Huh, in a weird way, it feels like the International version is actually the fixed/"patched" version, since revisions of games in those days were stupidly rare outside of Street Fighter.

Timesplitters 2 did it really well, where the level would change based on difficulty. Not only enemies, but also additional missions and stuff to do after the level ended on an easier setting (e.g. actually having to protect the dam)

I suck at Goldeneye-likes (perfect dark, timesplitters) but if I go to a lower difficulty the level ends halfway through :(

I remember long ago that Street Fighter 2 (not sure about the updates) on the SNES did the same thing.
I forget how it treated you on the easiest difficulty, but anything under Very Hard (8 stars) gave you the character ending and game ends.
To see the fancy end credits, you had to beat it on that last difficulty level.
You also got a fancy picture for no continues (the 8 fighters) and for not losing a round (8 fighters plus the 4 bosses) IIRC. Don't recall if that was tied to 4 stars or higher or you needed to be at level 8.
