I bought it in Japan (along with Yakuza 1+2 HD) and I'm playing it on my PS3. I just got to Chapter 5 and it is a little rough playing it in Japanese, especially now that the game opened up, but it's still manageable.

My thoughts so far:
-Definitely feels like a pre-Yakuza 3 game >_> The combat is a little jank (especially unarmed), and you have to load into an arena before you fight anyone on the street (like in Yakuza 1 and 2), instead of an instant transition to fighting like in Yakuza 3 onwards. Otherwise it still plays pretty well and you get used to the controls quickly.
-You play as Musashi Miyamoto and he goes under the pseudonym Kazumanosuke Kiryu. But he definitely doesn't act like modern Kiryu at all lol. He's constantly asking to get paid to do things, is kinda bloodthirsty about straight up killing people, and just casually asks courtesans "Yo, you wanna fuck tho?" LOL
-As for why this game didn't get localized, I can see why >_> The game does start out with Haruka asking Kiryu to kill a man that killed her family. When Kiryu asks for money to do it, she decides to sell herself to some high-ranking lady to get trained as a courtesan (aka high class prostitute) to get the money. I guess kids getting trained as courtesans was the norm in 1600 Japan? >_> They don't show anything at least. Also just like Ishin, this game is aggressively Japanese and just assumes you know who Musashi Miyamoto is and know the history of Japan in the 1600s, which 99% of Americans aren't familiar with. You definitely need to read up on the man and the times to get more out of the story.
-Speaking of the story, it's pretty good so far. I've been watching all the cutscenes translated on youtube as I play and getting context from the only GameFAQs guide, but I'm enjoying it a lot. I definitely want to see how events and plot points play out as I encounter them
-I kinda like the setting here more than Ishin so far. There's way more nature and mountain paths you traverse through here and an overall more diverse environment. Plus the 2-sword style is pretty fun and cool looking >_>

I'm playing this super casually every few days, so don't expect any porntastic posts like with my other Yakuza topics lol. I only know of 2 horny moments from this game: Kiryu wanting to fuck some prostitute and a training minigame where Kiryu meditates under a waterfall and imagines a dominatrix woman stomping his balls during it LOL

I do wish this game got localized because it does seem like a fun game
