This is somewhat correct. The lyrics are indeed disturbing despite being claimed to be written from the perspective of a 14 year old boy. Don't get why he bothered to change that as it makes it worse. And that one line sure is a doozy which should've seem disturbing even 40+ years ago.
But they don't reference a real life situation as much as you think and that model on the cover isn't underage.

The lyrics are referencing the relationship between the then 25 year old singer and his 17 year old girlfriend Sharona which is indeed a bit weird and even more so these days. But it's slightly less disturbing than picturing a guy in his mid 20s "running down the length of her thighs" of a barely 14 year old girl.
He and Sharona met in 77 when she was 17. The song got recorded and released in '79. Meaning the photo on the cover which is the actual Sharona is her at the age of 19.

EDIT: Listening to it again. Apparently it's running down the length of his thighs and not hers.

Also, the song wasn't in the movie, just on the soundtrack. Not that it makes the song better or worse, but a lot of people attribute the song to being in the movie when it actually wasn't.
