Mix of 2, 3, and 5

Many won't launch due to lack of proper maintenance, some at launch sites will refuse the command, and those that do successfully launch will be intercepeted. Any headed toward major population centers will be intercepted before they hit their target.

Now what happens to Russia afterwards is a whole other story. While it's almost a sure bet NATO and Co would return nukes, i do think there might be a non-nuclear reponse if the above happened and no nukes successfully hit target (read: civilian casualties were minimal).

Considering we've heard their nuclear sabre rattling for years now, its hard to imagine we don't have every known and unknown system monitoring all launch sites 24/7 and would know the moment they do that a nuke launched. As for their nuclear subs, it's also hard to imagine the location of every Russian nuclear sub isn't known 24/7 (normally, but especially now)

Following above, I'd expect a full force military invasion with America throwing the gloves off so hard they land in the nearest black hole and utterly eviscerating Russia's Navy and Air Force in under 72 hours.

It'd be the actual end of Russia as we know it.

I think that someone in the military would pull out a gun and shoot Putin in the head. Seriously. Because whether the nukes go off successfully or not, like Wackyteen said, this would be the end of Russia

In a purely hypothetical situation where the Russian nuclear arsenal is unleashed then it's the apocalypse.

Russia has an amusing history of old tech giving false positives and their chain of command stopping launches quite a few times, but these were automated systems. With the assumption it's a direct order from the top and a gun is literally to everyone's heads, and they launch, no city or strategic location will be missed. Our deterrent/defense systems for ICBM's are rapidly overstated and the world will get pancaked.

I think he would be assassinated, along with anyone else willing to carry out or authorize that course of action. Russia would never survive the retaliation the western world would throw at it and the non-deluded high rankers in Putin's regime would be well aware of that.

The only ambiguity to me would be wether or not his usurpers take credit for it, distance themselves from the Putin regime and it's goals or play it off as another "Sadly, our glorious leader fell out of a cup of tea" tragedy and maintain the current goals of the regime but with a less unstable leader.
