I mostly believe it, especially in anything where the situation is relatively complex. just because most people probably can't really bring themselves to care

I mean.... it's a nice expression but it's not meant to be a "universal truth" it's one of those sayings you use in response to someone that goes out of their way to assign nefarious intent to things.

Not never. Generally, and give anyone you don't know better the benefit of the doubt. As you get to know someone however, it can get to the point where you should attribute it to malice.
