Sexuality in the ancient world is complicated, it's not as black and white as people like to think it is. With a figure like Alexander the Great it's complicated because our surviving sources on him all come from centuries after his death and from people more interested in telling a moralizing story. Like Alexander taking the Persian king's former eunuch as his lover, Roman era writers might have used that as a means of showing Alexander becoming more like a Persian monarch.

In regard to homosexuality in a Macedonian context we can piece something out of contemporary sources. For example the assassination of Philip II being connected to love affairs he was having with his bodyguards, or the fact that Macedonian families sent their sons to serve the Macedonian court as a means to cement personal ties with the royal family. A Greek contemporary of Philip accused Philip's court of having bearded men having relationships with each other, among other "immoral" things, in a Greek context homosexual relationships outside of pederasty, a boy of around 13-17 and an adult man usually in his 30s, was seen as weird, but not unheard of in a Greek context, while in Macedonian royal culture it might have been normal for adult men to be in a relationship.
