No, just call them Mr./Ms./Mrs./Dr. whatever

if you insist on adding -san, make sure NOT to use Mr/Ms/Mrs at the same time

No it wouldn't. You only refer to god and emperors and the like with -sama. Doing so to anyone else would just make them uncomfortable.

I work at a Japanese company in Japan. You use -sama for customers/clients, whether by email or face to face
