Probably a dome, white ceiling, white ball, it can be insanely easy to lose and keep your eye on. This is very uncommon, but it happens.

Same can be said for any sport honestly, goalies let in weak goals from center ice, football players fumble or toss interceptions, NBA players miss free throws. Sports are imperfect.
