Many of these involve death scenes, shocking twists, or something just as major, however, they can also be as simple as the right imagery, tone, and music coming together - for the sole purpose of working those tear-ducts.

Grisaia no Kajitsu / A certain hospital scene. The first time I cried in a game, I was utterly destroyed emotionally. It detailed the un named's girl's final moments, before she became Michiru's other personality. After the accident, she was completely paralyzed, but could still see and stare at the ceiling. She couldn't do anything, or communicate with her parents in anyway :(
