If somebody is close enough that I expect that they'd reach the door before it closed behind me, I'll hold it. If not, I won't. That's generally good enough to avoid the awkward "holding for way too long" situation, without being rude about it.

Yeah I almost always do, but I hate when the person is really super slow and then when they finally go through it they don't even say thanks.

Yes. As long as they are like super far away or something. And as long as they are moving toward the door. If someone is stopped and talking to someone else, then K wont. Even if they are a little far away, I usually will do something like waiting a second before opening the door by either messing with something in my hand, or looking at my phone. Same with if I open the door already, but they are a little far
