When you use it in conversation, I mean. Do you define it via time, for example at midnight today becomes tomorrow or whatever, or are you like me and mean when you wake up?

I use wake up, but I know people who use midnight and then act all confused when I don't use it their way.

We use "tomday", which means technically today but we're not asleep and therefore it doesn't count. Basically the time between 12-4 AM

To me, tomorrow means "the next day that is relevant." So if I'm teaching a class, and it's Friday, and I say "your homework for tomorrow is..." and some smartass points out that "tomorrow's Saturday," I promptly remind him of my definition of tomorrow. Tomorrow's Monday, dumbass.

And then I realize I've forgotten that Monday is a holiday...

Well, say you're out drinking with some friends, you want to ask if they have any plans for the next following period of daylight, do you say tomorrow regardless of if it is before or after midnight, or do you say in the morning if it's after midnight?

I usually try to avoid saying anything about "tomorrow" at certain times. I'll usually say "later today" or "after x time." In an e-mail or text, I'll usually say the day of the week if I don't think they will read it until the morning. Stuff like that.

I prefer going strictly by time, so I'd use "today." If I'm still awake at 1 am and I'm talking about something I'm doing at noon the same day but I still have to sleep, I'll say that it's something I'm doing "later today."

I'm not confused by people who would say tomorrow, though.

My belief is after midnight, but most of my friends interpret it differently, so I just use their definition in conversation typically.

After midnight. It's the only way for it to be at the same time each day. Unlike something like morning or night which is changing based on the sun.

Also, why would 12:00 be noon/mid day if the day did not start at 12:01?

Waking up. There are exceptions of course, if I would go to bed at 8-10 AM after a particularly late party, I would use "later today" instead.
